Thank you for coming to talk to share some of your experiences and views associated psychiatry. I am sorry I have not emailed you sooner.  This was my first year taking three sciences and when it came time to study for finals, I pretty much ignored everything except school.

Thank you for giving the class and myself some exposure to psychiatry. Your talk was very intriguing, especially how you see a person, in particular how their brain health affects their personality. When you started to talk about homelessness it brought back painful and confusing memories from my childhood.

My parents divorced when I was seven and the majority of my time was spent with my mother because my now deceased father had a difficult time keeping a roof over his head and doing the activities of daily living. When I was a little older I even loaned my dad some money when his car was impounded. I could not understand why my dad was in the situation he was in and why I was seemingly more capable than him. I loved him very much and wanted to help him in any way I could.  But in the end, I could not make any of his decisions for him.

My older brother is living in a shelter and he reminds me of my dad in so many ways. When we lived together, before my parents divorced, my brother was just about as hard to get along with as my dad, and my dad was physically abusive to him. I was so confused and could not understand why we could not love each other or ourselves. My dad’s incessant fear of doctors and my brother’s fervent choice to self medicate only complicated the situation we were in.

My heart goes out to my family and people like them and I have a strong desire to help people. Your short talk resonated with me and I was intrigued by psychiatry because I thought it might be a way for me to help. How do I learn more?

Fred didn’t remember his panic.  He thought his main problem was his sleep.  His
so-called “main problem” changed with his symptomatology.  Fortunately or unfortunately he didn’t know it was happening.

Fred reminded me of a paper doll.  Now I’m a veterinarian, now I’m a clerk.  Of course there are all the stories that accompany each outfit.  Our smithy imagination is fast.  Pull this off and press this in and now I’m a fire-fighter.  Now I’m a noble, now I’m a… patient.

The other day after the Hemet NAMI meeting, (they meet monthly on the first Wednesday at the Hemet Seventh-Day Adventist Church), a member told me that when they do outreach, they begin their stories with something like, “We are people who,” or “I am a person who,” deliberately avoiding the word, “patient(s.)”  Hoping to allow others to connect with their humanity, the specialness of their, “Me,” rather than the distortion that suffering is special they try to keep away from the paper doll experience.

Thinking of NAMI, thinking of Fred, I splayed the biopsychosocial-model tools I use.  What was here for Fred?  Fred’s biology was toward healing as he wasn’t having panic attacks any more and his thought processes were less circular.  That’s what we wanted and signified that his treatments, (including medications and psychotherapies,) were at least not harming him as far as we could tell, and might even be part of what influenced his healing process.  However, his ongoing symptomatology as seen in his poor insight, (paper-doll syndrome,) insomnia and persistent worrying thoughts demonstrated that his biology was only partially treated.

Fred, like you and I, and like women who labor babies into this world never remember their pain, by forgetting his panic, he lost his point of reference.  I said,

Fred!  This is significant!  Yay!  

Fred looked at me like I didn’t get it.  He wasn’t sleeping.  What was I thinking, “Yay?”  Well…  “Fred I was thinking you aren’t panicking on a gurney in the emergency-room today.  Yay.”

Remembering our suffering isn’t necessary but it can be a friendly reference point if we want.

Self-Care Tip:  Use previous suffering as a reference point to celebrate when you aren’t.  Be a friend to yourself.

Question:  Have previous sufferings lost their strength in your memory and diminished your celebrations?  How has suffering been used after they are gone to your advantage?  Please tell me your story.

Connect With Others to Get Friendly With Yourself

Self-Care Tip #81 – Connect with others.  Be a friend to yourself.

So you have bought into the famous, “You are not alone” stock.  After 2 months on psychotropics (medications for emotional illness,) you finally have an interest in people.  You are at least a little motivated and less afraid of things that move.  You don’t feel like you are the reason for original sin and more often than not, you think happiness might be more than what shopping can offer.  What is this strange and unfamiliar sensation?  And what to do with it?

It is time to connect.  Many of us get to the point where we no longer want to hide, we don’t hate ourselves, and we don’t hate others.  We get to the place of showing our under-belly just a little to the big wide world and are shocked that the only thing we feel is the wind as everyone is rushing by!  Just when we start wanting what we spent so much time hiding from, we seem to have forgotten how to connect with others.

It is no secret.  America is culturally impoverished.  We have little of cobblestone streets to meander down, dressed in clean clothes after a days work, checking up on neighbors and gossip.  We have few degrees of activity between full throttle and dead/no heart beat.   Come now!  How to connect in a world where our parents expected us to pay rent when we turned 18years old?

If you find yourself in something of this situation try on one of these basic tools and see what fits.  You can’t expect them all to.  So if you strike out a few times, keep on!

1.  Volunteer – for example, and in no particular order…

2.  Meetup.com – an awesome site to find people interested in what you are interested in.  e.g. book clubs, skiing, small business, Italian

3.  Support groups

4.  Write!  Although this at first thought may appear isolating, it is not necessarily.

Self-Care Tip #81 – Connect with others.  Be a friend to yourself.

Claim Your Brain

My mind, like rusted gears, was not moving well.  It hadn’t been really since my 1st pregnancy 8 years ago.  There are few things that dumb us down as much as pregnancy and children!   Hormone changes, lack of sleep, fluctuating from 145-200-145 pounds three times, and then the subsequent growing beloveds around me to contribute to mental dissociation.  Simple sensory overload from talking, yelling, crying, petitioning, inquiring kids factors in.  You may read more about sensory issues here.

Daily writing, like a staunch governess, found my brain under cobwebs, bug carcasses, and musty stench.  (Hello old friend!  There you are!)  This helps to explain the joy gripping my hand, like girlfriends on the playground, when I sit down to write!  The world is active to me, including rather than excluding me. My in-between moments used to hang like an old woman’s breasts.   Now much more time full of nourishing thoughts bless me.  I am in awe.

My patient came in sighing deeply.  He wasn’t better.  No, he said.  He lacked motivation and interest and connection from the world.  He felt selected out to suffer.  A dumping ground for misfortune and misunderstood.  Efforts through medication, after medication changes were like looking for love in all the wrong places.

We talked about cognitive distortions, tapping into things that used to make him happy, road-blocks in poorly designed neurological grooves – volunteering at the library or animal shelter, journaling, sharing his life story with others, exploring his spirituality.  No.  No good.  Nor could he consider psychotherapy as he’d been through too much of it already to consider it again.  And he just couldn’t get interested in groups such as through NAMI.

His brain, assaulted by stressors, disease, and disuse was growing silent.

Being a friend means yelling, fighting to reclaim your journey, finding something to connect you to your process of life.  My patient was letting squatters take his property simply by being absent.

Self Care Tip #60 – Claim your right to health.  Be a friend to yourself.

Good News

Many people see needing to take medication as bad news. But I think about what it would be like without it. Suicide, progressive deteriorating processes in the brain biology, contagious behaviors and moods spreading to those you love, inflammation…. That is bad news. I think about the not so many years ago before most of our medications existed. Before much of our understanding about the brain biology was around. Those times were hard. Misinformed people had ugly ways of looking at others with emotional illnesses. Hearing someone thump out their opinions on the pulpit about human behavior has always been a pleasure for me as well – not! Now we know that our essence isn’t dependent on our brain biology.

But here we are, in the land of milk and honey, depressed economy and all. We have a more informed public opinion (check out NAMI – awesome!), evidenced based medications, etc…. More than ever before in our history, the responsibility to take care of ourselves comes down to us as individuals. The external barriers to treatment are not what they used to be. However, what are the internal barriers? We own our choices. Our beliefs are our own. Letting yourself close off to the good news of medication – that is a tragedy.

Now is the time to fight for yourself. You are worth it. When you see the difference in your life, your perspective on good news and bad news might change a little too. Even public opinion starts with the individual.
